Cassandra Toscano, MBA, has extensive experience in population health and healthcare management. Cassandra is currently working as the Vice President of Population Health Program Excellence & Medicare Care Management at AmeriHealth Caritas, where they are part of a leadership team focused on achieving population health goals and enhancing members' lives. Prior to this, Cassandra served as the Interim Population Health Leadership and Subject Matter Expert (SME) at PWC Talent Exchange, where they worked on various projects related to clinical program design, advocacy program design, and Medicare D-SNP Duals care management program leadership.
