Pat de Leon-Hoffman

Pat De Leon-Hoffman has extensive experience in the hospice care sector, serving as a Hospice Liaison at Apreva Hospice since February 2015, and previously with Sonata Hospice from April 2013 to January 2015. Prior roles include Hospice Liaison at Interlink Hospice & Home Health from 2011 to 2013, and as Area Director of Sales at Odyssey Hospice/Gentiva in 2010-2011. Pat's career in the hospice industry began in 2000 as a Community Education Representative at Odyssey Hospice until 2010, and earlier as a Community Liaison at Hospice By The Sea in 1995-1996.
