Grace Murphy

Grace Murphy has extensive experience in the field of social work, currently serving as an LTSS Care Coordinator at Anthem since September 2017 and as an LCSW at Ascend Management Innovations since October 2011. In the role at Ascend, Grace writes Level II PASSR assessments, conducting interviews to produce psychosocial and medical evaluations that inform Medicare recommendations for client independence and address various psychological and physical issues. Previously, Grace worked as a Clinical Social Worker at Vanderbilt Psychiatric Hospital from 2003 to 2008, providing individual and family therapy and managing discharge planning. Grace's career began at the Sexual Assault Center in Nashville, TN, from 1996 to 2003.
