Dr. Leigh Ann Farmer

Dr. Leigh Ann Farmer, DM, MSN, RN, NEA-BC, NI-BC, CCM, CSSBB, serves as the Director of Care Management and Population Health at Johns Hopkins Health Plans, overseeing six service areas and ensuring compliance with regulatory standards. In addition to current responsibilities, Dr. Farmer functions as a faculty member at Aspen University and as President of LA Consulting, providing expertise in clinical operations and quality improvement. Previous roles include contributing faculty at Walden University, Clinical Subject Matter Expert at Altruista Health, and various leadership positions at Blue Cross and Blue Shield of Illinois and Aetna.
