Courtni Boozer MSW has a diverse work experience in the field of social work. Courtni started their career as a Case Management Intern at Communities In Schools, where they prepared needs assessments and treatment plans for students. Courtni then interned at Seton Medical Center as a Social Work/Case Management Intern, where they provided social service interventions and assisted with discharge planning.
After their internship, Courtni worked as a Rehab Clinician at MHMRA of Harris County, providing mental health recovery-oriented services to individuals with severe mental illness. Courtni also served as a Service Coordinator at The Harris Center for Mental Health and IDD, where they provided service coordination for individuals with intellectual disabilities.
Courtni then transitioned to employment counseling and case management roles. Courtni worked as an Employment Counselor at Neighborhood Centers Inc., assisting customers with career advice, job searching, and placement. Courtni later became a Life Skills Coach/Housing Case Manager at Houston Area Community Services, providing services and information to program participants to promote job skills and stable housing.
More recently, Courtni worked as a Mental Health Case Manager at Avenue 360 Health and Wellness, where they performed comprehensive psychosocial assessments, developed treatment plans, and ensured linkage to mental health and substance abuse services. Courtni also served as a Risk Reduction Specialist at Change Happens, focusing on reducing risk behaviors.
Throughout their work experience, Courtni has demonstrated their passion for helping individuals improve their well-being and achieve stability.
Courtni Boozer MSW received their Bachelor of Science (B.S.) degree in Social Work from The University of Texas at Austin, where they studied from 2007 to 2012. Following their undergraduate studies, they pursued a Master's degree in Social Work Direct Practice at Texas State University, which they completed in 2014.

In November of 2016, Houston Area Community Services and Bering Omega merged to join forces as a Federally Qualified Health Center (FQHC) under the new name Avenue 360 Health & Wellness.The new nameis a representation of a literal avenue—a path one takes to get to their destination.
