Melanie Poole

Melanie Poole is a dedicated healthcare professional with extensive experience in patient advocacy and care coordination. Currently serving as a Patient Navigator/Care Coordinator at Brentwood Hospital since August 2021, Melanie assists patients in determining post-discharge options such as Partial Hospitalization and Intensive Outpatient Therapy. Since June 2013, Melanie has also been a Verbal De-escalation certified instructor. Previously, Melanie held the position of Patient Advocate and Nurse Navigator at Brentwood Hospital beginning in April 2011, receiving the UHS Service Excellence Award in 2015. Additionally, Melanie worked as a Physician Liaison for PrimeSTAR Home Health from June 2020 to August 2021 and has a background as a former nurse. Education credentials include an LPN degree in Practical Nursing from Northwestern State University and additional schooling at Southern University at Shreveport and LSU Shreveport.
