Dawn Warner joined Capital City Nurses Healthcare Services as a Nurse Care Manager in March 2013. Dawn is responsible for providing direct patient care and overseeing nonskilled and skilled nursing services for patients with acute, chronic or complicated medical conditions. Dawn obtains medical histories and performs comprehensive physical assessments with regular and as needed home follow-up visits. She collaborates with patients, families and the healthcare team to develop a plan of care, delegates and assigns interventions to implement plan of care and then evaluates and supervises personnel to ensure quality of care. Dawn also manages long-term care plans for patients , sets up and maintains medication schedules and coordinates, attends and transcribes notes for medical appointments. In her various roles, Dawn advocates for the well-being of her patients and helps provide customized care by educating families on community and healthcare resources.
Prior to joining Capital City Nurses Healthcare Services, Dawn served as a bedside nurse for 23 years on Medical, Surgical, Infusion and Oncology Units. She started her career as an Army Nurse at Walter Reed Army Medical Center and advanced to the rank of Captain until she was honorably discharged after 4 years.
Dawn has 4 children and enjoys spending time with family, walking and bargain hunting.
