Rachel Woods

Rachel Woods is a seasoned medical social worker currently with Concord Hospital Health System, having previously served as a hospice social worker at Granite VNA. Rachel's diverse background includes social work internships at JSSA and the Housing Opportunities Commission, where case management for individuals experiencing homelessness was a key focus.
