Cardeum

Cardeum was founded on the idea that patients should rarely require readmission to the hospital. For Medicare alone, the estimated annual cost of hospital readmissions is $26 billion, of which $17 billion is considered avoidable. Our vision is a world with zero avoidable hospital readmissions, and we deploy a suite of products and services for our clients in pursuit of that goal. If you are a health system, hospital, or home care agency interested in reducing avoidable hospital readmission in 90 days or less, then we are interested in you. Let’s get to work
