Autumn Akers, QMHA NCMA, is a seasoned healthcare professional with extensive experience in community health and case management. Currently employed at CareOregon since August 2015 as a Community Healthcare Worker and Community Health Outreach Worker, Autumn facilitates access to preventive services and coordinates care through the food RX program. Previous roles at Old Town Clinic/Central City Concern include Clinic Case Manager, where Autumn supported individuals experiencing homelessness with healthcare navigation, and Panel Manager, overseeing a team of medical assistants. Earlier experiences as a Medical Assistant at both Old Town Clinic and Legacy Good Samaritan Resident Clinic provided foundational skills in patient care and support. Autumn holds a degree in Medical Assisting from Concord Career Institute.
