Welcome to Celebrating Life Community Health Center (CLCHC), founded in 2018, CLCHC preserves the legacy of Kristina Sergeyeva, daughter of the founder and CEO Sergey Sergeyev, who passed away unexpectedly at the young age of 24. CLCHC is HRSA designated Federally Qualified Health Center (FQHC) look-alike health center located in Mission Viejo, Costa Mesa, Irvine, and Santa Ana, CA, with service to the surrounding community. FQHCs, such as CLCHC, are safety net providers that primarily engage in providing medical, dental, behavioral health, and social services in their outpatient clinics. The primary focus of the FQHC program is to enhance the provision of primary care services in medically underserved communities.
