Healthcare and Clinical Support

The Healthcare and Clinical Support team at the Center for Comprehensive Health Practice (CCHP) provides integrated and holistic care to address the physical, emotional, and social needs of patients and their families. Comprised of licensed clinical social workers (LCSWs), a certified rehabilitation counselor (Ms.Ed, CRC), and a nurse practitioner, this team collaborates to deliver high-quality primary care, substance use treatment, behavioral health services, and patient education. Their efforts ensure comprehensive, family-focused healthcare for East Harlem and surrounding New York City communities, regardless of race, ethnicity, gender, sexual orientation, religion, age, or ability to pay.
