Jade Smallwood currently serves as a Patient Services Coordinator at Centerpoint Health, where responsibilities include advocating for patients and connecting high-risk individuals to essential community services. Previous experience includes roles as a Case Manager at Community Health Alliance, where Jade provided mental health support and assistance with social services, and as a Case Manager at Shelterhouse, focused on enhancing clients' well-being and housing solutions. Jade has also contributed to the community as a Volunteer Leader with Young Life, mentoring high school girls, and as an Assistant Volleyball Coach with Middletown City Schools. Early career experiences include writing and media management at Miami University and internships with the City of Middletown Health Department and VITAS Healthcare. Jade holds a Bachelor's degree in Nonprofit & Community Studies from Miami University.

Centerpoint Health was created through a collective effort to improve health in the region. This effort was led by a large group of caring individuals representing the residents of Warren County. Participants included the healthcare community, social service organizations, local government, schools, and private businesses who wanted to make it easier for people to access high quality, affordable healthcare locally.
