Case Management and Client Services

The Case Management and Client Services team at Circle of Care, Sinai Health, works collaboratively to assess client needs, develop personalized care plans, and coordinate a variety of home and community-based services. This team ensures that clients, including Holocaust survivors, those needing hospice care, and individuals facing urgent needs, receive tailored support. They also manage transportation services and provide specialized social work and bereavement services to enhance the quality of life and independence of clients in their home environments.
