Circle of Life Community Hospice

We provide excellent clinical care and spiritual support to relieve your physical pain and help you to find peace at the end of life. Our Hospice Team: Our Medical Director and Hospice Physician supervise the team and review each patient’s case on a regular basis. They also provide visits when medically necessary. Our Nursing Staff will provide care and support to both patient and their caregivers with regularly scheduled visits throughout the week. They provide training and medications for symptom management and pain relief. Nurses are also available 24/7 to answer any questions you may have and for visits as needed should there be a change in the patient condition. Our Certified Nursing Assistants provide personal care and help with grooming, bathing, changing linens and limited household needs. Our Medical Social Workers are knowledgeable in all aspects of community services and resources. They provide education regarding advanced directives and help with forms and written materials. They also provide emotional support and assist with ongoing patient and family needs. Spiritual Care is provided by our Chaplains only when requested. Their relationship with you is not predicated upon any religion or cultural beliefs. It is one of unconditional presence and compassionate caring. They will try to help you develop an inner emotional and spiritual peace. Our Bereavement Support Program is open to everyone in the community. Our goal is to provide support thought grief education, group settings, creative workshops and individual sessions. Trained hospice volunteers provide companionship to patients, and respite to family members and caregivers. Volunteers come from diverse backgrounds with many talents. They are available to support the unique needs and interests of each patient.
