Columbia Basin Care is the region’s only independent, non-profit, skilled rehabilitation and long-term care facility. 

Our team of over 100 employees provide around-the-clock medical care for over 60 residents. Our on-site Geriatric Nurse Practitioner serves as a primary care physician, providing consistent and immediate medical attention. An expert team of physical, occupational, and speech therapists are conveniently in-house to provide comprehensive rehabilitation and restorative services.

Set on five serene acres, Columbia Basin Care is located within Columbia Gorge National Scenic Area and in the small-town city of The Dalles, just over an hour east of Portland. Here on Oregon's "dry side,"​ the sun shines over 300 days a year, and outdoor adventures — hiking, biking, fishing, boating, and more — are everyday opportunities.

The Medical Directors Team at Columbia Basin Care oversees the clinical and medical care of all residents, ensuring the highest standards of healthcare are met. They are responsible for coordinating with the on-site Geriatric Nurse Practitioner and various therapy teams to develop and implement individualized care plans. Additionally, they ensure compliance with healthcare regulations and maintain detailed medical records to support the continuity of care and rehabilitation services.
