Case Management Team

The Case Management Team at Columbus Medical Services, LLC works collaboratively to coordinate and deliver individualized care plans for individuals with intellectual or developmental disabilities (I/DD). Their responsibilities include assessing client needs, facilitating access to necessary services and resources, monitoring ongoing progress, and advocating for the well-being and rights of their clients.
