The Communi T mobile application aims to enhance support delivery,  health equity, human services and Enhanced Care Management services  through a collaborative and integrated approach. The app will serve as a central hub for  community health workers (CHW/P/Rs), community doulas, mental health peer supporters, and domestic violence peer advocates to coordinate care,  provide telehealth and in home services.
