For over 20 years, Community Care has been caring for individuals and families across the country. We are a home health care agency offering both medical and non-medical services in the greater Smithtown, White Plains, Nanuet, Rochester, and Buffalo areas. Our team consists of companions, live-ins, home health aides, nurses, and more, providing support services that allow clients to remain independent at home.

Our Companions can:
•	Prepare meals and assist with feeding
•	Grocery shop
•	Assist with pet care
•	Engage in gaming, crafts, reading, and more
•	Drive to appointments and personal  engagements
•	Provide socialization, friendship, and support
•	 Remind clients to take their medications
•	Assist with mobility
•	Run errands
•	Provide companionship and conversation during hospital stays

Our highly skilled Aides can:
•	Perform personal care services (bathing, grooming, etc.)
•	Assist clients with their daily routine
•	Prepare meals according to special diets and schedules
•	Assist with walking. mobility, and supportive devices
•	Escort clients to appointments
•	Care for bed bound clients
•	Perform light housekeeping tasks
•	Assist with prescribed exercise routines
•	Remind clients to take medications

Our private duty Nurses can:
•	Prepare medications weekly
•	Visit monthly for wellness checks
•	Monitor vital signs
•	Provide diabetic teaching & management
•	Manage wound care
•	Administer tube feedings
•	Provide dressing changes
•	Administer injections
•	Communicate with physicians and medical team
