Community Care Plan
Jeimy Ariza, BAS, LSSGB, is a Continuous Improvement Specialist at Community Care Plan since August 2020, where responsibilities include data collection for process improvement initiatives, tracking progress, and conducting audits to ensure sustained improvement. Previous roles include Project Coordinator in the Enterprise Project Management Office and Global Health Specialist at Memorial Healthcare System, managing patient services and international patient care. Earlier experience encompasses positions at Cleveland Clinic and Dennis A Cortes MD, focusing on patient scheduling and financial services. Jeimy Ariza holds a Bachelor of Applied Science in Management and Supervision and an Associate’s Degree in Health Services Administration from Miami Dade College, and studied Business Administration at Universidad Colegio Mayor de Cundinamarca.
Community Care Plan
Community Care Plan (CCP), 'the health plan with a heart,' was introduced in 2000 as South Florida Community Care Network (SFCCN), the first safety-net hospital-owned Provider Service Network. CCP serves 100,000+ members enrolled in Medicaid, Florida Healthy Kids, sponsored programs, and self-insured employee health plans. CCP is owned by Memorial Healthcare System and Broward Health. Community Care Plan maintains accreditation and has earned Multicultural Health Care Distinction for its Medicaid Health Plan by the National Committee for Quality Assurance (NCQA). CCP is also fully accredited by the Accreditation Association for Ambulatory Health Care (AAAHC) as a Health Plan. MISSION To promote healthier communities.
