Elizabeth Herzan-Taylor has extensive experience in nursing and healthcare, serving in various capacities as a Nurse Practitioner over several years. Currently, Elizabeth is a Volunteer Provider-NP at CommunityHealth since August 2023. Prior to this role, Elizabeth worked as a Family Nurse Practitioner at Near North Health Services from October 2013 to December 2022. Elizabeth's career includes positions at Rush University College of Nursing and CommunityHealth, as well as VNA Community Healthcare and Chicago Commons Comprehensive Senior Care, where roles included clinical management. Elizabeth holds a Master of Science in Community Health Nursing from St. Xavier University and a Bachelor of Science in Nursing from the University of Illinois Chicago.

Founded in 1993, CommunityHealth is dedicated to serving the uninsured and underserved in Chicago and surrounding communities. We provide over 17,000 medical and dental visits for more than 10,000 patients every year. This level of care is made possible thanks to the time and talent of more than 1,000 volunteers, including 350+ providers (physicians, nurses, nurse practitioners, dentists, pharmacists, counselors, etc.). CommunityHealth’s main facility, the Lederman Family Health Center, is located in Chicago’s West Town neighborhood. In 2010, we opened a satellite in Chicago’s south side Englewood neighborhood — a community with great need and limited resources. As the largest volunteer-based free medical facility in the United States, we offer demonstrated success and an innovative model for providing compassionate and comprehensive medical and dental care to the uninsured.
