Daniel Lozoya, MSW, LCDC-I, QMHP-CS, currently serves as a Collaborative Care Specialist at Concert Health since May 2022, engaging with patients through video or telephone to set goals, provide counseling, monitor medications, and assess symptoms. In addition, Daniel has worked as a Community Living Support (CLS) provider for the Center for Health Care Services since November 2020, and previously held roles including Service Coordinator I at Superior HealthPlan and MSW Advanced Student Intern at Crisis Text Line. Daniel's extensive experience in clinical services includes a tenure as Senior Care Manager and Senior Clinical Practitioner at the Center for Health Care Services, as well as providing community support for children with severe mental illness. Daniel earned a Master of Social Work from Our Lady of the Lake University and a Bachelor of Arts in Psychology from Texas A&M University-Corpus Christi.
