Deitra Harris

Deitra Harris, LCSW, LISW-CP, FSW, currently serves as a Collaborative Care Clinician at Concert Health since April 2022, where the focus is on developing rapport with patients and guiding them in skill development for relapse prevention. As the Owner and Therapist at Evolution You, PLLC, since April 2018, Deitra provides online and community-based therapy. Previous roles include working as a Social Worker for U.S. Renal Care, where duties involved improving patient health and coordinating kidney transplant referrals, and as a Case Manager MSW at Novant Health Presbyterian Medical Center, focusing on discharge planning for complex cases. Extensive experience includes positions at Davita Healthcare, Dialysis Clinic, Inc., and Renal Ventures Management, where Deitra contributed to psychosocial care and implemented significant interventions to enhance patient outcomes. Deitra holds a Master’s degree in Social Work and a Bachelor’s degree in Psychology, both from West Virginia University.
