Jade Flagg

Jade Flagg, LSW, is a dedicated social worker with extensive experience in case management and client advocacy. Currently serving as a Social Worker Care Coordinator at Cook County Health since August 2021, Jade manages assessments and case management for over 100 clients, focusing on conflict resolution and crisis de-escalation. Previously, Jade held roles including Child Welfare Specialist at the Illinois Department of Children and Family Services and various case management positions within organizations such as WINGS Program, Inc. and Catholic Charities of the Archdiocese of Chicago. Educationally, Jade earned a Master of Social Work from Dominican University and a Bachelor of Arts in Communication and Media Studies from Columbia College Chicago.
