LeAnn Chapman is an experienced registered nurse with a diverse background in various healthcare settings. Currently serving as an RN Case Manager at Eastern Plumas Health Care since October 2014, LeAnn oversees all aspects of care for long-term care residents while assisting LVN and CNA team members. Previous roles include working at Quincy Home Medical Services, where LeAnn managed acute and chronic diseases, and at Eastern Plumas Health Care in multiple capacities, including emergency and skilled nursing, clinical supervision, and outpatient services. Earlier experience includes high-risk labor and delivery nursing at California Medical Center. LeAnn holds an Associate of Arts and Sciences degree in Nursing from Glendale Community College and has training as a medical assistant from Bryman School of Medical Assisting.

Eastern Plumas Health Care is a small, non-profit, critical access hospital district, providing comprehensive medical services to Plumas County since 1971. We operate a 9 bed acute care hospital at our main Portola campus, which includes a 24 hour, physician staffed, emergency room and ambulance service. In addition, four primary care medical clinics and a dental clinic offer residents a full spectrum of health care services in the Graeagle, Greenville, Loyalton, and Portola communities. There are also hospital-based Skilled Nursing Facilities in Portola and Loyalton. The hospital district serves over 10,000 permanent residents living in small communities scattered throughout Plumas and Sierra Counties. During the peak summer vacation months, that population almost doubles. In good weather, the commute to larger Reno and Truckee hospitals is 45 minutes. During the winter months, these roads are sometimes impassable, making EPHC one of the most vital services in our region. When necessary, critical care flight services can transport patients directly to Reno. We also provide in-patient and out-patient surgery, diagnostic imaging, dietary, and respiratory services, hospice care, and visiting specialists.
