Jake Bryan, OT, MOT-L, currently serves as the Executive Director at Eden Health, where responsibilities include overseeing home health, hospice, and home care services. Jake also owned and managed Sandcreek Family Practice and Avalon Home Health and Hospice, ensuring comprehensive medical care and strategic operational success. Early roles include serving as Business Owner at Integrity Home Health and Hospice and Director of Rehabilitation at Mountain Land Rehabilitation.
