Caritina Almanza

Caritina Almanza, MSW, serves as the Referral and Training Manager at Medical Home Network since April 2022 and has held the position of Community Health Worker Manager at Enlace Chicago since November 2018. Prior experience includes the role of Patient Care Manager at Alivio Medical Center, Inc. from May 2017 to November 2018.
