Erin Crooks-Martin MHA, MS, RN, HCS-O, HCS-C has extensive work experience in the healthcare industry. Erin Crooks-Martin is currently serving as the Chief Operating Officer at Enliven Home Health since October 2022. Prior to that, they worked as the Regional Director of Home Health Operations at Trinity Health At Home from January 2020 to October 2022. Erin Crooks-Martin also held the position of Director of Clinical Operations - Home Health at Mount Carmel Health System from September 2017 to October 2022.

Erin has also worked at Synergy Home Health where they served as the Administrator / Home Health Agency Director from January 2014 to September 2017, and as the Director of Clinical Services from January 2009 to January 2014. Erin Crooks-Martin gained experience as a Clinical Supervisor at CarePartners Home Health from January 2008 to December 2008. Prior to that, they worked as an RN / Nurse Case Manager at The Marden Companies from 2005 to 2007.
