Carolyn Phan Pt, Dpt

Carolyn Phan, PT, DPT, is a dedicated physical therapist with diverse clinical experience. Currently employed at Evolution Physical Therapy since March 2023, Carolyn previously served as a Clinical Specialist Resident at Kaiser Permanente from February 2022 to February 2023. A comprehensive education includes a Doctor of Physical Therapy degree from Creighton University (2018-2021) and a Bachelor's degree in Kinesiology from California State University, Fullerton (2011-2016). Carolyn has applied skills in various settings, including outpatient, inpatient, and skilled nursing facilities, while focusing on patient-centered care for individuals with neurological disorders, post-surgical needs, and musculoskeletal conditions.
