Jacob Hope

Jacob Hope is an experienced professional in health policy and outreach, currently serving as the Health Policy & Follow Up Team Lead and Health Policy Specialist at Fabric Health since March 2023. Jacob also operates as a consultant for Hope Community Health Consulting LLC, providing expertise on PA DHS programs and offering training and support on outreach and enrollment best practices. Previously, Jacob led the Outreach and Enrollment program at The Family Practice & Counseling Network from September 2015 to March 2023, managing staff and maximizing revenue recovery. Jacob's early career includes a role as an Outreach Organizer at the National Family Planning & Reproductive Health Association, where engagement strategies improved clinic payer mix. Jacob holds a Bachelor of Arts in History and Politics from Ursinus College and has additional educational experience from American University.
