fwd collective is a team of sole trader Specialist Support Coordinators/Social Workers supporting people with complex needs, high‑risk presentations and justice involvement.

Servicing the Geelong and West Melbourne region, we provide Level 3 Specialist Support Coordination with a trauma‑informed, culturally safe and relational approach.

We work alongside participants, families, justice services, mental health, AOD and clinical teams to stabilise risk, improve engagement, and help individuals navigate complex NDIS pathways. Our practice is grounded in expertise, collaboration and respect.

We specialise in:
• Justice‑involved participants
• High‑risk or unstable circumstances
• Complex trauma & psychosocial disability
• Multi‑agency coordination
• Housing instability or homelessness
• Challenging transitions and disengagement
• Participants in hospital, custody or secure environments

Our approach:
• Client‑led
• Strengths‑focused
• Culturally safe
• Evidence‑informed
• Compassionate and relational

If you’re a clinician, justice worker, AOD team, hospital service, or allied health professional needing Level 3 specialist input, we can step in quickly and support the pathway forward.
