Case Management Team

The Case Management Team at the Georgia Department of Human Services is responsible for assessing, planning, and facilitating personalized care plans for individuals and families in need. They coordinate with various divisions and external partners to ensure clients receive the necessary support and resources to achieve self-sufficiency, safety, and well-being. Team members include Case Managers and Court Liaisons who work collaboratively to address and manage individual cases efficiently.
