Grow provides quality services to adults with intellectual and developmental disabilities. We presently have four programs: • Day Habilitation is a day program for the more medically fragile. Day Hab is designed to meet health, daily living, skilled nursing and therapeutic needs. Day Hab offers activities and therapies based on individualized day habilitation service plan (DHSP) which sets forth measurable goals and objectives to develop and maintain skills. Most important, the people we serve value their friendships. their friendships among Grow’s diverse community. • Community Based Day Supports (CBDS) is a day program that integrates individuals in the community as much as possible through a variety of classroom activities, community outings, and volunteer opportunities. These activities help the people we serve maintain skills for Activities of Daily Living, social skills, and to reach other individual goals. • Vocational / Group Supported Employment (GSE): Individuals in this category work for us in this building on our staff and we help them with skills we hope will lead them to more independent employment • Integrated Employment Services: When an individual wants to work in the community, they receive training and on-the-job support from Grow. The people Grow serves have access to Job Coaches and Case Managers that help them set goals and build skills - a Career Development Plan to find jobs that are a perfect fit. In addition, Grow’s fleet of vans pick up many of the individuals we serve and take them home at the end of the day. It is an essential service that provides access to Grow, community outings, and employment.
