Jennifer Powell BS, MSW, MBA-HCM has a diverse work history in the healthcare industry. Jennifer began their career as the Manager of Clinical Resource Management at Detroit Medical Center, where they provided oversight for various processes, including case management and utilization review. Jennifer then served as Interim Director of Clinical Resource Management, overseeing the Hospice Program at Sinai-Grace Hospital.

Jennifer went on to work at CHE Trinity Health as the System Director of Clinical Integration and Continuing Care. In this role, they played a vital role in planning and integrating continuing care services within the Clinically Integrated Networks and Accountable Care Organizations.

Jennifer then joined US Medical Management as the Vice President of Medical Management, where they implemented a national integrated care management model for the homebound senior population. Jennifer led Transitions of Care programs and was responsible for improving quality and community-based support programs.

Jennifer's most recent role is as the Vice President of Palliative Care and Hospice at HarmonyCares. In this position, they administer and coordinates activities in all hospice and palliative care operations and works with the leadership team to develop and implement agency programs.
