HN is an award-winning company that uses AI to predict and prevent care need. We provide practical applications of population health management, going beyond simply identifying high-need patients to proactively intervening and supporting them to improve their health outcomes.

Established by clinicians and researchers at Sweden’s Karolinska Medical Institute, our journey in the UK began in 2015. Working in collaboration with the Nuffield Trust, we launched a large trial into the effectiveness of clinical coaching, which has shown exceptional outcomes for patients and the NHS. 

Since our UK launch, we have developed into an award-winning NHS partner and since March 2020, have been supported by the NHS Innovation Accelerator to scale nationally. 

The HN approach is unique. We blend our advanced data scientific and AI capabilities with digital services, which are delivered by highly experienced healthcare professionals. This provides the NHS and patients with integrated and comprehensive solutions.
