Director Population Health Clinical Program at Health New England

Maggie Perracchio, RN BSN has extensive experience in the healthcare industry. Maggie most recently worked at Health New England as the Director of Population Health Clinical Program, starting in December 2019. Prior to that, they served as the Vice President of Clinical Programs - CT, NJ & PA and coordinated the Regional Care Group at Prospect Medical Holdings, Inc. from January 2017 to December 2019. In this role, they were responsible for developing, implementing, and overseeing various clinical programs.

Before joining Prospect Medical Holdings, Inc., Maggie worked at ConnectiCare, where they held several director positions. From October 2015 to December 2016, they were the Director of Total Population Health. From November 2013 to October 2015, they served as the Director of Medicare Care Management. In addition, they were the Director of Clinical Integration from December 2012 to December 2013.

Maggie's work experience demonstrates their expertise in population health, clinical programs, case management, care transitions, quality management, utilization management, HEDIS/STARS, and clinical integration.

Maggie Perracchio, RN BSN, began their education at Albertus Magnus College from 1980 to 1984, where they pursued a Bachelor's degree in Biochemistry. Later, they attended the College of Our Lady of the Elms from 1985 to 1987 to earn another Bachelor's degree. During this time, they focused their studies in the field of Registered Nursing/Registered Nurse.
