Gabriel Kalmuss-Katz

Gabriel Kalmuss-Katz is an experienced professional in community health and social services, currently serving as Manager of Community Health Programs and Homeless Clinical Coordinator at Health Partners Plans since October 2021. Prior roles include Assistant Team Leader and Service Coordinator at Pathways to Housing PA from June 2017 to September 2021 and Reentry Case Manager at the Goldring Reentry Initiative from August 2015 to May 2017, where responsibilities involved supporting individuals transitioning from the Philadelphia Department of Prisons.
