Thuong Pham

Thuong "Terry" Pham, RN, BSN, BA, is an experienced clinical nurse navigator currently working at Hoag Hospital since August 2015, specializing in transitional care for pneumonia patients to minimize preventable readmissions. Prior roles include RN Clinical Case Manager in the Sub-ICU, where multi-complex care coordination and discharge planning were prioritized, and interim perioperative services clinical RN manager overseeing the GYN, urology, and plastic surgery departments. Pham has also served as a clinical resource nurse and operating room surgery circulator with a focus on various surgical specialties. Previous experience includes work with Banner Health as an operating room RN and as a freelance artist and community service specialist, managing project support and financial oversight in education.
