Jeffrey Stratton is a seasoned healthcare professional with extensive experience in hospice nursing and healthcare IT consulting. Currently serving as a Hospice RN at Hospice & Community Care since June 2017, Jeffrey has been dedicated to providing compassionate care to patients with life-limiting illnesses since March 2015. Previous roles include Hospice RN Case Manager at PruittHealth, where responsibilities involved delivering direct care and coordinating interdisciplinary services, and Registered Nurse at Hospice Care of South Carolina, focusing on patient comfort and quality of life. Prior to nursing, Jeffrey worked as a Sr. Healthcare IT Consultant and Implementation Consultant at McKesson Health IT, showcasing expertise in project management and healthcare technology. Jeffrey holds a Bachelor of Science in Experimental Psychology from Presbyterian College and an Associate of Science in Nursing from York Technical College.

Hospice & Community Care's mission to provide hospice services and give hope, comfort, and compassion to our community when needed most. Our interdisciplinary team includes board-certified physicians, hospice-trained registered nurses, and certified nursing assistants, licensed social workers, bereavement counselors, and spiritual care leaders. The team focuses on quality at end of life by addressing the physical, emotional, social, and spiritual needs of patients and their loved ones. We have a 16-bed inpatient hospice house and also serve patients where they live; whether in their own homes, a nursing home, assisted living, or other facility.
