Autumn Hill MSW, LSW is an experienced social worker and educator with a strong background in clinical and medical social work. Currently serving as an Adjunct Faculty member at Central Ohio Technical College since January 2021, Autumn has also worked as a Medical Social Worker at Hospice of Central Ohio since August 2017, and previously at Generation Hospice and Generation Home Health Care. Autumn's career began with a role as Social Service Director at Autumn Health Care, Inc., and includes positions such as Therapeutic Mentor at Mid-Ohio Psychological Services and Math Tutor at The Ohio State University. Educational qualifications include a Doctorate in Social Work from Tulane University, a Master’s degree in Clinical/Medical Social Work, and a Bachelor of Science in Social Work from The Ohio State University.

Our patients have been the center of care since 1982 by offering personalized care and support to thousands of patients across Central Ohio. We believe that everyone should have access to end-of-life care regardless of their ability to pay, complexity of care, or severity of need. Working together, we can ensure that all eligible patients have a meaningful end-of-life experience and the opportunity to write the last chapter of their life regardless of the their disease, and pledge to provide access to our staff and programs 24 hours a day/seven days a week. Our team consists of highly dedicated and trained individuals that include physicians, registered nurses, hospice aides, social workers, chaplains, bereavement counselors and trained volunteers. These individuals formulate a specific plan of care for each of our patients and their loved ones special needs. Our patients have the opportunity to receive quality, compassionate care at home, in a skilled nursing facility, assisted living facility, hospitals, our Inpatient Care Center, or anywhere the patient calls home. Hospice of Central Ohio is Medicare/Medicaid certified and is covered by most insurance companies/carriers.
