Johanna Berberena, LMSW, is an experienced social work professional with a diverse background in healthcare and social services. Currently serving as a Hospice Clinical Liaison at Hospice Of New York since May 2019 and as a Licensed Social Worker at Stamford Health since July 2018, Johanna also holds the position of Transitional Care Coach at White Plains Hospital since April 2013, where skills in Motivational Interviewing are utilized to support patients in modifying unhealthy behaviors to prevent hospital re-admissions.
