Kristi Y. has extensive experience in the healthcare field, with a focus on hospice and palliative care. Kristi started their career as a Staff Nurse and Relief Charge Nurse at Adventist Medical Center in Portland in 2003. Kristi then transitioned to Columbia View Family Health Center, where they worked as a Clinic Manager for three years. In 2007, they joined Adventist Hospice as a Hospice RN Case Manager and later became the Hospice and Palliative Care Team Lead from 2010 to 2013. In 2014, Kristi joined CareOregon as the Advanced Illness and Palliative Care Manager, where they worked until 2017. Currently, they hold the position of Director of Clinical Operations at Housecall Providers.
Kristi Y. obtained their Bachelor of Science degree in Nursing from Walla Walla University in the years 2001 to 2003. Kristi also holds additional certifications as a Certified Hospice and Palliative Nurse (CHPN) from the Hospice and Palliative Nurses Association, obtained in 2013, and as a Registered Nurse from OSBN, obtained in 2003.

Housecall Providers is a Portland-area nonprofit organization that believes everybody should have the right to medical care, delivered at the right place, at the right time. We offer team-based primary care wherever you call home and hospice services to be with our patients throughout the course of their lives. In addition, we provide community-based palliative care support for seriously ill members enrolled in Medicaid through our advanced illness care program. Together, all three of our programs are proving that in home medical care for seriously ill patients can improve health outcomes, increase patient comfort, reduce health care costs and contribute to care team satisfaction.
