Leslie Cook, BSN, RN, CCM, currently serves as a PRN Inpatient Case Manager at St. Luke's Health, overseeing Med-Surg and Acute Care floors since August 2022. Simultaneously, Leslie fills the role of Substitute School Nurse at Hudson ISD, providing coverage across various campuses. Prior experience includes serving as a Medicare Advantage Care Manager for Catalyst Health Group, managing care for over 15 independent primary care practices in East Texas. Leslie also worked as an RN Service Coordinator for UnitedHealth Group, focusing on Texas STAR Kids, and held positions as an Outpatient Surgery RN and Critical Care RN at Woodland Heights Medical Center. Earlier in the career, Leslie was a Critical Care RN at Nacogdoches Memorial Health. Leslie earned a Bachelor’s Degree in Nursing Science from Stephen F. Austin State University.
