Welcome to Human Connections ! We are a community-based disability support program in the great State of Oregon that strives to do things a little differently! We are tired of the circular motion of ISP’s, goals, provider supports, etc. It’s time to shake things up for real forward progress!

We provide services with PURPOSE. We individualize all aspects of our supports, ensuring the focus stays on the HUMAN at the center of it all. Our goal is to braid together teams, meet goals and find creative ways to assist our clients in growing past barriers. Everyone we work has maintains their own unique strengths and qualities.
