Eva Rizzo

Eva Rizzo, MSW, LCSW, currently serves as the Integrated Transitional Care Manager at IEHP and as a Medical Social Worker with Regal Medical Group. With a background that includes roles at Charter Healthcare, Catholic Charities San Bernardino & Riverside Counties, and Family Solutions, Eva has honed specialized skills in psychosocial assessment, patient advocacy, and care coordination. Eva's experience encompasses developing therapeutic rapport, addressing mental health concerns, and facilitating interdisciplinary team meetings. An innovative community-focused strategist, Eva has successfully spearheaded projects enhancing community well-being and has made significant contributions to social work practices through in-service facilitation and professional development initiatives. Eva holds a Master of Social Work from California State University-San Bernardino and a Bachelor's degree in Psychology from California State University, Los Angeles.
