Holly Swirkowski

Holly Swirkowski has extensive experience in care management and social work, currently serving as the Manager of Care Management Support at Inclusa Family Care, a subsidiary of Humana Inc., since December 2023. Previously, Holly was a Manager of Care Manager Support at Inclusa from January 2017 and has held various roles in member support coordination and management since 2011. Holly's background includes significant work at Community Care of Central Wisconsin, where the focus was on supporting members with disabilities and frail elders. Holly's early career involved social work positions in multiple organizations, including North Central Health Care and Langlade County Department of Social Services, emphasizing assessment, care planning, and support services for vulnerable populations. Academically, Holly is a Certified Social Worker and holds a degree in Human Development from UW - Green Bay and is pursuing Nursing Home Administration at UW - Madison.
