LIFEPlan is a Care Coordination Organization (CCO), formed in 2018 along with six other CCO’s in New York State to meet the needs of people with intellectual and developmental disabilities by collaborating with service providers through a Health Home model. CCOs are staffed by Care Managers with training and experience in the field of developmental disabilities.

LIFEPlan CCO is a partnership of community-based agencies with a long history of serving people with I/DD, providing advocacy, recreational, educational and vocational services. We share a person-centered focus that improves services and supports through networking with highly trained human service, healthcare and other specialists.

Together, they collaborate to drive organizational goals, enhance service delivery, and maintain strong partnerships with community-based agencies and service providers, all while fostering a person-centered approach.

The Engineering and Technology team at LIFEPlan CCO is responsible for the development, implementation, and maintenance of technology solutions that support care coordination for individuals with intellectual and developmental disabilities. This team ensures the integrity, security, and efficiency of health information systems, employs data analytics to enhance service delivery, and provides network support to ensure seamless operation within the organization. Their work enables the integration and utilization of advanced technologies to improve person-centered care and support services.
