Jessica Arcoraci is an experienced nursing professional with a diverse background in care management and population health. Currently serving as a Care Management Supervisor and Population Health Manager at Maryland Primary Care Physicians since April 2019, Jessica oversees a panel of over 30,000 patients, focusing on the stabilization of high-risk individuals through effective transitions of care and resource coordination. Prior roles include Medical Case Manager at Genex Services, where Jessica managed care for injured clients, and Clinical Case Manager at Natick VNA, providing oversight for a multidisciplinary team.
