Eric Guitron

Eric Guitron currently serves as a Partner Success Specialist at Medication Management Partners since September 2022, focusing on building relationships with community partners to improve resident quality of life and optimize service value. Previously, Eric worked as a Community Health Worker for AMITA Health from May 2021 to April 2022, where responsibilities included evaluating patient social risks and developing a diabetes care program. From January 2017 to May 2021, Eric held the position of Service Delivery Manager for OnePoint Patient Care, managing day-to-day interactions with hospice partners. Eric obtained a high school diploma from De La Salle Institute in Chicago, IL.
