Heather Foucht

Heather Foucht currently holds the position of Director of Quality Improvement and Director of Value Based Care at Modivcare, starting in January 2023. Prior to this role, Heather served as the Executive Director of Quality and Director of Clinical Resource Services at Alternate Solutions Health Network from September 2015 to January 2023. In these positions, Heather provided support and oversight for agency population health teams, collaborated with health system partners on key clinical initiatives, aimed to close care gaps, and focused on improving patient outcomes.
